Learn how to define, identify and address social determinants of health (SDoHs) which impede the prescribed care and management of your patients. Gain knowledge from a public health practitioner, researcher, and educator on the fundamental ways to collect, report and address SDoHs while mitigating cost through the collaboration with community-based resources. The presenter will explain how to reduce missed opportunities during patient engagement to appropriately assess the needs of patients to improve health and health outcomes.
Dr. Allysceaeioun D. Britt is a public health practitioner with 20 years’ experience and research in healthcare administration and policy for Medicaid populations, program development and evaluation, population health engagement with a focus on social determinants health and vaccine-preventable diseases. She currently serves as Interim Vice President of Faculty Affairs and Development and Assistant Professor in School of Graduate Studies and Research at Meharry Medical College, Nashville TN.
